A 23" waist is not that tiny. 20 years ago, the average woman was 38 24 36. It's just that there are a lot of 'big' girls now whose waists are usually 36-40" in circumference.
That's large for a man.

Hopefully the obesity epidemic will end, but not at the expense of the bewbies!!
